How do I collect anonymous feedback?

Place an anonymous feedback box on any webpage and allow students or others to provide comments, concerns, or suggestions completely anonymously. The service is hosted externally so their contact and IP information is kept secret. Contributors can optionally provide an email address to receive a response, but messages are relayed so that the email provided is also kept secret.
